1^st – 5^th Grade Choice Requirements

Student currently enrolled in school must be in good standing at their current elementary school, which means:

The student comes to school on a consistent basis (no more than 9 unexcused absences, no more than 9 tardies, this year)
There are no discipline/behavior issues on file for the student.

If applying for grades 1st - 5th, please answer the next two questions.
